I use FF since Netscape died and never I have seen those three dots. I really don't need those dots at all since I use an external screenshot applet. Anyway, I guess the answer is in the page you suggested Chris. (copy/pasted): "Don't see Screenshots? We are still testing this feature so not everyone will see it. Note: Firefox Screenshots currently will not work in Private Browsing. We are working to fix this for future releases." BTW, I don't use Private Browsing.

However, none of the icons appear in the Address Bar if the user has Firefox configured to display the default Firefox Home page. I just tested that with a new profile. I noticed that you have your tabs under the toolbar. I suspect you have an add-on or a userChrome script that is hiding the page actions menu. If you restart Firefox in Safe Mode, does the menu appear?
